Woke Up with a Stiff Neck? Here's Why
Sleeping position is rarely the real cause. We look at what actually triggers overnight neck pain and the fastest way to get moving again in the morning.
Waking up with a locked, painful neck is a surprisingly common and surprisingly misunderstood problem. Most people blame their pillow or their sleeping position. Occasionally they're right - but more often, the overnight episode is a symptom of something that was already building during the day.
What usually causes it
The cervical spine is most vulnerable to becoming irritated when it's held in one position for a prolonged period. Prolonged desk work, looking down at a phone, or sitting with the head forward all create sustained load on the posterior neck structures. By the end of the day, the joint capsules and muscles are already sensitised. A few hours of sleep in a slightly awkward position is enough to tip it over into an acute flare.
What to do in the first hour
Resist the urge to stay completely still. Gentle, slow range-of-motion exercises - turning the head side to side and nodding up and down to the first point of resistance - are more effective at reducing stiffness than rest. Applying a heat pack to the neck and upper trapezius for 15 to 20 minutes alongside this is also helpful.
Getting through the day
Avoid driving if rotation is significantly restricted. Do small movement breaks every 30 to 45 minutes throughout the day. A soft cervical collar is not recommended - it promotes deconditioning and slows recovery. If the pain is severe, anti-inflammatories and muscle relaxants prescribed by a GP can help in the short term.
If it keeps happening
Recurring overnight neck pain is a signal that something upstream needs addressing - usually a combination of thoracic stiffness, weak deep neck flexors, and poor workstation setup. A physio assessment can identify the specific contributing factors and put a plan in place to stop it coming back.
